A faxed and signed complaint counts as a formal complaint, which obligates OSHA to consider an on-site inspection rather than just a phone follow-up. Tip: Spell out the workplace address, the employer's name, and a detailed description of the hazard, since vague complaints are harder for OSHA to act on. You can file anonymously, and OSHA will not disclose your identity to the employer.
